Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: 10 ppm chlorine sanitizer detected in three-vat sink; corrected to 100 ppm chlorine and measured with a test strip provided by facility.

July 22

La Huerta

1270 A U.S. 412, Suite A, Siloam Springs

Critical violations: No bare-hand contact with food or ice; use gloves, spatulas, tongs, deli tissue or other dispensing equipment (corrected). Dish machine is not sanitizing at all; employee has to prime the machine after each use; machine should not have to be primed. Lettuce and cheese are being held at room temperature; must be maintained at 41 degrees or below. Beans in walk-in are 50 degrees; may have not been cooled properly or long enough prior to putting in walk-in. Milk and salsa in small cooler not at 41 degrees or below. Large container of salsa is held at 41 degrees or below.

Noncritical violations: Items stored below fajita meats are not covered (corrected). Standing water where flooring is cracked in the handwashing sink area (corrected).

Mazzio's Pizza

Critical violations: Some of the salad bar items are not covered in the refrigerator.

Noncritical violations: No sanitizer at three-compartment sink (corrected). Ceiling tiles in food preparation are are dirty; clean or replace. Some ceiling tile is cracked and falling down; replace as needed. Ice buildup in freezer; condensation leak. Buildup on the interior of the exhaust fans above the oven; clean all the way to the roof and the unit on the roof. Water coming through cracks in the floor in various locations; plumber is scheduled to come in tonight to repair the problem. Contain food and equipment during work; clean and sanitize all surfaces and equipment after work is completed.
