Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Employees need to spend more time lathering when washing hands (corrected). Maintain hot food at 135 degrees.

Core violations: Thawing food in pans of warm water at room temperature. Employee not rinsing dishes prior to putting in sanitizer. Employee using a cloth towel to dry dishes. Employee not wearing effective hair restraints. Food employee wearing jewelry. Do not double pan hot/cold food items, it can prevent proper holding temperatures.

March 5

Priority violations: Items not date marked properly or not at all. Grill drawer is not holding food at 41 degrees or below; items moved to another cooler.

Priority violations: No date marking on item at pizza station. Chicken legs reheated as needed; maintain hot food at 135 degrees of above. Employee washed hands with disposable gloves on (corrected). Foods not cooled properly in walk-in.

Priority violations: Small prep table (fajitas) is not holding all food at 41 degrees or below. Employee handled ready-to-eat food with bare hands (corrected). Maintain salsa at 41 degrees or below. Some pans of raw meat stored above other foods in reach-in three-door cooler (corrected)

Core violations: No thermometer in fajita prep table. Lid broken on one chest freezer, insulation is exposed; repair or replace. Containers of uncooked rice and beans are uncovered; keep food storage containers covered when not in use. Clean interior of beer cooler. Floor peeling in various places in the kitchen.

IHOP

Priority violations: No sanitizer detected in final rinse of dish machine; pump hose had come loose (corrected); pump hose tightened.

JJ's Grill n Chill

Priority violations: No chlorine residual detected after running empty dish machine (corrected); sanitizer primed; 50 ppm chlorine residual.

March 13

Embassy Suites

Priority violations: Employee handling ready-to-eat foods with bare hands. Cooler by stove is not holding food at 41 degrees or below. Food on serving line is not date marked. Sliced turkey on sandwich station is above the load line and is 47 degrees (corrected). Some items in produce cooler are not at 41 degrees or below. Demonstration of knowledge out of compliance. Banquet service station water fill nozzle laying in sink; keep fill nozzle hanging up or out of sink.

Core violations: No paper towels at handwashing sink (corrected). Need an alternative method to check the heat sanitizing cycle on both kitchen and bar dishwashers.

Core violations: No thermometer in refrigerator (corrected); need a thermometer for freezer. Effective hair restraints are required for food employees.

Flash Market

998 S. Lincoln St., Siloam Springs

Priority violations: Hot case is not holding all food at 135 degrees or above. Items not date marked as needed. Biscuits are not at 41 degrees or below. Boxes of lettuce and tomatoes on floor in walk-in cooler; food must be stored at least six inches off floor (corrected).

Core violations: Prepackaged grab and go food items must be labeled with ingredients. Ceiling tiles must be smooth and easily cleanable in food preparation areas. Hole in tile; repair or replace as needed. Replace broken light shield in food prep area. Walls in food preparation area are no longer in good condition and not easily cleanable. Wall behind sink in women's restroom is no longer smooth and easily cleanable. Sink in men's room needs to be recaulked; water is getting in around sink.

Angeline's Italian Restaurant

2001 Highway 412 E., Siloam Springs

Priority violations: Salad items uncovered in walk-in (corrected). Tea dispensers are not covered (corrected). Spray bottles not properly labeled with contents or mislabeled spray bottles.

Core violations: Clean interior of dishwasher, especially the drain well. Condensation leak/ice buildup in both walk-in cooler and walk-in freezer.
