Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Time is not being marked on the ready rack; pizzas at 57 to 61 degrees (repeat). Spray bottles of chemicals facing ready-to-use pans (corrected).

Core violations: Boxes stored on the floor in walk-in cooler. Open box of cookies and box of wings under fans in walk-in; condensation buildup on fan unit could drip onto open boxes; do not store any open items under condensation. No soap at back handwashing sink. Broken handle on the hot water side of the faucet.

Priority violations: Volunteer employees not washing hands between glove changes or when gloves could be contaminated. Ensure hot holding unit stays on or plugged in if the unit will be used. Only scheduled volunteer workers are allowed in the concession stand.

Core violations: Cover/shield light bulbs or use shatter resistant bulbs. Paint ceiling in the new room. No test strips for sanitizer.

Little Sunshine's Playhouse

3468 W. Pleasant Grove Road, Rogers

Priority violations: Spray bottles of bleach water are not labeled with the contents; all rooms.

Core violations: Replace bowls used in microwave soon as they are melted or bubbly and no longer smooth (corrected).

E-Z Mart

1350 S. Main St., Cave Springs

Priority violations: Employee does not know if establishment has sanitizer for dishes or not. Employee does not know how to make sanitizer solution. All food equipment must be washed, rinsed and sanitized after use. Corn dogs must be cooked to 160 degrees internal temperature; product only reached 152 degrees; cook until 160 degrees.

Core violations: Broken thermometer in hot box. Clean and defrost freezer. Sanitizer on location; no test strips. All employees must be taught how to make sanitizer and how to test the concentration. Clean under shelves in walk-in. Clean interior of milk cooler.

Ridgeview Group Home

12958 Highway 112, Cave Springs

Priority violations: Food is not being held at 41 degrees or below in front fridge. Raw meat stored above/next to ready-to-eat food (corrected).

Core violations: No paper towels for drying hands in handwashing sink area (corrected). Upper portion of the counter top is separating/peeling and exposing the wood underneath. Mop water being dumped outside in the yard; all mop water must go through the sanitary sewer. Pump the grease trap frequently. Front of handwashing sink needs to be sealed/repaired.

The Creeks Golf Course

1499 S. Main St., Cave Springs

Priority violations: No date marking on hot dogs. Maintain cold food at 41 degrees or below; adjusted.

Core violations: Install a handwashing sink in the food preparation area. Remove unused or unnecessary equipment.

Core violations: Scones on self-service are not covered; must be protected from contamination during display.

Northwest Arkansas Children's Shelter

14100 Vaughn Road, Bentonville

Core violations: Condensation leak in freezer. Monitor dishwasher chemical concentration in girl's wing.

McDonald's

8055 Regional Airport Blvd., Bentonville

Priority violations: Employees not washing hands prior to putting on gloves. Sanitizer at three-compartment sink is too strong; should be 200 to 400 ppm.

Core violations: No thermometer in the frappuccino machine. Front handwashing sink does not seem to have cold water; water temperature is 121 degrees.

Wingate Inn

7398 S.W. Old Farm Blvd., Bentonville

Priority violations: Ensure food is reaching proper heating temperatures prior to putting in hot holding.

Core violations: Replace light bulbs in kitchen; lights flashing. Establishment does not have a thermometer to test food temperatures.
