Critical violations: Some employees did not wash their hands as needed, but most employees washed their hands; employees who did not wash their hands did when asked to, an improvement from previous inspections. Some raw meat stored above celery and shrimp in walk-in (corrected). Some items on the buffet not at 135 degrees or above, items on wire rack are not holding temperature; remove wire rack so food can come in contact with the pan directly for better heat distribution (repeat violation). Not all items in the wok prep table were at 41 degrees or below; sausage was at 54 degrees, meatball was at 43 degrees and chicken was at 63 degrees. Do not store food on top of other pans, food will not stay cold, food was destroyed as needed, unit was adjusted as needed. Lettuce and cole slaw are not being held at 41 degrees or below, ice is not around the food to keep it cold; when using ice the bowl must be surrounded by ice up to the level of the food, more ice was added. Tub of cream cheese sitting at room temperature (corrected). Items not date marked as needed; food not used in 24 hours must have a date mark on the container (repeat violation).

Noncritical violations: Fish and chicken were at room temperature. No thermometer in the wok prep table. Effective hair restraints are required for all employees. Knives stored between the preparation tables; between the tables is not clean so knives are getting dirty (repeat violation). Dish machine over flows soap; contact dishwasher repair person to determine what the cause may be. Some liquid spilled in the walk in freezer, clean freezer floor. Threshold in both the walk-in cooler and freezer is bent and not providing a good seal; food and water getting under them and cannot be cleaned properly. Tile grout around dish area has standing water; fill grout in as needed to allow for easy cleaning and prevent standing water.

Hammontree's

113 W. Walnut St., Rogers

Critical violations: Employee with gloved hands cleaned prep tables with wash cloth and did not remove gloves to proceed to prep bread, must remove gloves and wash hands before putting on new gloves and when changing task such as cleaning before proceeding to food prep.

Noncritical violations: Some soda boxes for fountain drink are being stored under sewer pipes in the basement; food should not be stored in prohibited areas. Employee bathroom door is open; must have self-closing door to keep shut during food prep.

In-use utensils must be cleaned and sanitized every four hours or less if not held at the same temperature as the food. Not all items on the steam table are at 135 degrees or above (adjusted). Items in front hot box are not at 135 degrees or above (adjusted). Prep table is not holding food at 41 degrees or below (adjusted).

Noncritical violations: Condensation leak and water leaking from the cooler unit in the walk-in with a pan under leak to catch water; do not store food under any portion of unit until the unit is repaired. Floors, walls and equipment need to be cleaned, excessive grease, dirt and food debris build-up on most surfaces in the kitchen and food storage area and excessive dirt and debris under and around three-compartment sink, especially the drain (repeat violation). Drain under three-compartment sink is not draining properly (repeat violation). Floor around three-compartment sink is no longer in good repair, a large portion of the concrete is missing; was repaired but floor is still holding water and is no longer easily cleanable; gasket torn on front hot holding unit and condensation leak in walk-in cooler. Permit is not posted in customer view.

Critical violations: Employee did not wash hands between task change when going back into the food prep area before working with food items. No hand washing sign at the hand sink. Milk in thermos is not at 41 degrees or below.

Noncritical violations: Effective hair restraints are not being worn by food employees. After cleaning and sanitizing, equipment and utensils shall be air dried and may not be cloth dried. Clean cups and glasses stored on cloth towel; clean equipment and utensils, laundered linens and single-service items shall be stored in a clean, dry location where they are not exposed to splash, dust or other contamination at least six inches above the floor. Seal the floor wall junction in both bathrooms, this was supposed to have been done prior to opening.
