Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: Cooling units were not working properly yet were being used. Some raw fish stored above plates of spinach dip. Sanitizer is not strong enough; call service provider. Salad prep table, prep table No. 2 (fish cooler) and grill drawers are not holding food at 41 degrees or below; raw fish, cooked foods and other items destroyed as needed; service provider arrived during inspection. Consumer advisory is on the menu, but not all foods are marked with an asterisk to identify foods requiring the advisory (repeat violation).

Noncritical violations: One thermometer is missing in the new coolers in the banquet room servers area; no thermometer in prep table No. 2. Double stacking pans of food on top of uncovered pan of mixed greens. Ice scoop servers station laying in ice. Knives stored behind the block of wood above the three-compartment sink; store clean and sanitized utensils in a proper location to prevent recontamination (repeat violation). Wood behind three-compartment sink is no longer in good condition and easily cleanable (repeat violation). No test strips at dishwasher in kitchen (previous violation). Curtain between kitchen and servers' area is not clean; clean as needed to prevent the buildup of dirt on the edges where employees touch to move it out of the way. Ice buildup on walk-in freezer door and door frame (repeat violation). Walk-in cooler floor joint is coming up off floor, secure as needed. Rusty shelves in the walk-in cooler. Leak in drain of handwashing sink by the ice machine.

Sunrise Cafe

Critical violations: Upright freezer is not freezing food solid so some raw meats stored above and next to ready-to-eat foods; because foods are not frozen solid there is a risk of cross contamination. Sanitizer is not strong enough (repeat violation). Potato slicer is not clean; excessive food debris stuck in the upper part of the slicer and bottom blade and wall behind it. Inside of cooler and prep table are not clean; bottom of cooler has excessive food; trays and containers in the unit are not clean. Refrigerator in the back prep area is not clean; the inside has food spilled and the exterior handle is not clean. Excessive food and grease accumulation around microwave, rice cookers, fryer, grill, under grill and around the stove burners. Excessive grease and food accumulation under all equipment. Foods in the upright refrigerator are not being held at 41 degrees or below; food in the prep table is not at 41 degrees or below. Food is not date-marked as needed (repeat violation). Bottle of liquid Tylenol leaking and dripping onto other items stored in and around the door of the back white refrigerator; products and other food items stored below must be destroyed.

Noncritical violations: Stored frozen foods shall be maintained frozen; upright freezer is not keeping food frozen. No thermometer in the prep table cooler. Using cardboard under microwave and rolling cart; cardboard is saturated with grease and food debris; grease saturated cardboard stored between the prep table and the cooler (remove; repeat violation). Walls and exterior of equipment, countertops and other surfaces have an accumulation of food and grease. Exhaust vent and filters have an accumulation of grease; contact a professional to clean the entire exhaust vent inside and out. Back bathroom opens into a storage and prep area and door must stay closed.

Critical violations: Cole slaw at 49 degrees in walk-in cooler; must be kept at 41 degrees or below.

Noncritical violations: Walk-in cooler at 50 degrees; not keeping foods at 41 degrees or below. Some floor tiles in walk-in freezer are missing; not a smooth cleanable, nonabsorbent surface.

Arby's

Critical violations: Sliced roast beef at 44 degrees and dressing at 44 degrees in prep two bottom; must be kept at 41 degrees or below.

Critical violations: Keep ice cream frozen; product was not frozen. Hot dogs opened with no date mark. Items must be stored to prevent contamination; ice cream scoop was on dirty counter.

Noncritical violations: Incorrect test strips for sanitizer in use at time of inspection. Freezer is out of service; has melted products in the bottom.
