Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

April 6

James Butts Baseball Complex

The Crumpet

5212 Village Parkway, Suite 12, Rogers

Critical violations: Complete cooling process of food items in the three-door upright cooler instead of the reach-in cooler; it is possible the heat from the quiche baked today and put in the reach-in cooler raised the temperature of the one stored above it, both were at 55 degrees, but nothing else checked in the same unit was above 41 degrees.

Noncritical violations: Pre-wrapped scones and other items displayed for customer grab-and-go must be labeled with name, ingredients and facility name; packaged food shall comply with standards of identity requirements. Server's ice scoop is stored on the chest freezer; store the scoop on or in something that can be cleaned and sanitized regularly. The wall under the three-compartment sink is not clean; clean it frequently to prevent the accumulation of grease and dirt.

Fuzzy's Taco Shop

4204 S. JB Hunt Drive, Suite 10, Rogers

Critical violations: Employees are not washing their hands properly, they did not lather long enough before rinsing soap; the employee also did not wash between task changes. The dish machine is not dispensing sanitizer; nozzle may have been clogged, or it is not holding a prime; the machine was primed and nozzles were cleaned -- check machine as needed to ensure machine is holding a prime. A pan of beef was on the line at 126 degrees and may not have been reheated enough prior to putting on the steam table; item was reheated as needed.

Noncritical violations: The floor in the main prep area is starting to peel and pit; the seam in the floor by the dish area is not sealed properly allowing standing water.

Rockin' Pig Saloon - Mobile

2407 W. Hudson Road, Rogers

Critical violations: Raw bacon, uncooked whole shell eggs above ready-to-eat food in main kitchen. Dish machine had 0 ppm chlorine. Cheese sauce mobile unit not at proper holding temperature. Pickled onions, sloppy sauce held more than seven days. Clear liquid in spray bottle had no label. No sanitizer test strips used in dish machine.

Noncritical violations: Meat on floor around dish machine, three-compartment sink and slicing area in main kitchen. Cracks and chips in concrete floor in main kitchen. Permit not posted.

Noncritical violations: Wall inside back walk-in door is rusty and no longer in good condition, repair or replace with materials that are designed for that application. Replace weather stripping around back exterior door. Gasket torn on kitchen walk-in door. Condensation leak in freezer, one unit beginning to freeze up.

Long John Silver's

Critical violations: Portioned cole slaw in the top part of refrigerator is at 43 degrees; must maintain at 41 degrees or below at all times. Line prep table is not holding food at 41 degrees or below, food temperatures vary from 48 to 56 degrees; unit adjusted and food destroyed as needed. Hot dogs are not date marked, repeat violation.

Noncritical violations: Area behind the fryer/steamer unit especially behind pipes has accumulation of grease and food. Tile missing at floor wall junction around the mop sink and the opposite wall, allows for stagnant water and cannot be cleaned easily; grout missing from tiles around mop sink area and under three-compartment sink. Excessive ice build-up around the walk-in freezer door, ice does not allow for door to close properly; floor in walk-in cooler is buckling.

Critical violations: Employee did not wash hands properly; lather with soap for 10 seconds before rinsing off soap; employee rinsed off soap before lathering. A handwashing sink must be maintained so that it is accessible at all times for employee use, and do not use for purposes other than handwashing; servers are dumping contents of glasses into the hand washing sink, sink is full of ice, straws and lemon wedges. No sanitizer concentration at three-compartment sink. Hot-holding foods not held at temperature of 135 degrees or above; cooked eel in toaster oven at 91 degrees, some items on buffet are not at 135 degrees. Date marking for potentially hazardous food prepared on side or opened commercial containers held for more than 24 hours. No time marked on sushi rice or sushi rolls; repeat violation. Reusing a glass cleaner or chemical bottle for unknown substance; contents emptied, bottle destroyed; spray bottles not labeled with contents.

Noncritical violations: Food not in the original packages, unlabeled bottles at the sushi bar area; food must be identified with common name of food. Food on display such as salad, salad dressing, cut fruit and tray of sushi rolls under heat lamp, must be protected from contamination from consumers; keep covers on sushi rolls when customers are not getting product. Reusing hot sauce bottles for other food items; use only containers that are designed to be durable enough to be washed and sanitized multiple times. Condensation leak in walk-in freezer.

Provide hand washing sinks as necessary for convenient use by employees in food preparation, warewashing areas and toilet rooms. Beef in meat case discolored, sausage in walk-in cooler laying against boxes and pallet uncovered, vegetables have brown spots and mold. Raw food stored above ready-to-eat food in bottom of prep table; food inside walk-in cooler has no covering. Food debris on knives. Chicharrons not being held at proper hot-holding temperature. Prepared foods in walk-in cooler lack date marking. Premises must be maintained free of insects, rodents, and other pests and controlled to eliminate their presence on the premises. Noncritical violations: Food not in the original packages for use in establishment must be identified with the common name of the food; tortillas not labeled or for retail sale on shelf, chicharron on shelves not labeled. Knives stored in container with tools. Outside of prep table had dry spices, food debris. Lids on trash cans broken.

Chen's Garden Buffet

Critical violations: Discontinue consuming food, use of unapproved beverage containers in food preparation or warewashing area; beverage containers without a lid or a straw in food prep area. A handwashing sink shall be maintained so that it is accessible at all times for employee use, and do not use for purposes other than hand washing; garbage container obstructing hand washing sink access.

Noncritical violations: Sluggish drain at handwashing sink requires attention for free flow.

Firehouse Subs

2009 Promenade Blvd., Rogers

Critical violations: Soap dispenser is not working correctly, soap is hard to get out of dispenser. Meatball and sauce was not reheated to 165 degrees before ready to serve. Meatball in sauce 128 degrees in hot well; must be kept above 135 degrees or above after cooking to 165 or above. Bacon at 44 degrees and butter at 43 degrees in glass door cooler by drive through, roast beef at 48 degrees, mayonnaise at 48 degrees, turkey slices at 48 degrees in prep cooler; must be kept at 41 degrees or below; turkey slices were sampled for bacteriological testing; potentially hazardous food in prep table discarded.

Noncritical violations: Glass door cooler by drive through at 44 degrees and prep cooler 48 degrees is not keeping foods at 41 degrees or below. Food prep employee did not have proper hair restraint.

April 19

Pinnacle Bar & Grill

5001 W. Pauline Whitaker Parkway, Suite 100, Rogers

Critical violations: Complaint inspection. Employee did not lather with soap long enough before rinsing off soap, employee did not wash the back of hand. Employees did not wash hands prior to food preparation. Employee handled kettle chips with bare hands; no bare-hand contact with ready-to-eat food is allowed, use glove, deli tissue, utensil or other approved method. French fry cutter is not clean and attracting gnats. No sanitizer in three-compartment sink. Alfredo in walk-in cooler at 98 degrees; cool food from 135 to 70 degrees in two hours or less, from 70 to 41 degrees in four hours or less. Raw meat in grill drawer unit above 53 degrees, shrimp at 60 degrees, unit thermometer at 53 degrees. Blu cheese bacon dip stored above load line, temperature of product at 57 degrees at the top. Pan of grilled chicken sitting on prep table cutting board at 77 degrees. Several items not date marked as needed. Several items exceed use by date or are marked incorrectly. Wrong sanitizer used to replace empty sanitizer at three-compartment sink.

Noncritical violations: No thermometers in prep tables. Scoop in bulk container stored in product. Food storage containers stored on the top of the ice machine bin; area is not clean for storing food containers. Interior of freezer/cooler unit is not clean. Dish area and under the three-compartment sink floor and wall is not clean. Gaskets on grill drawers are torn and no longer in good condition.
