Washington County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: Employee beverages on bread racks and prep tables, uncovered with no lids; must ensure no drinking/eating in food prep areas unless using protected cups. Handwashing sinks in prep area of front bar and in baking kitchen area not equipped with soap; must provide an adequate supply of hand cleanser (liquid, powder or bar) at each handwashing sink. Handwashing sinks in prep area of front bar and in baking kitchen area not equipped with paper towels; must provide a supply of individual disposable towels that supplies the user with a clean towel at each handwashing sink. Handwashing sink in baking kitchen area has dishes in sink basin; must ensure that handwashing sinks are not blocked or used for purposes other than handwashing. Food-prep tables and food-contact surfaces of equipment have debris/food/grease buildup; must frequently/effectively clean and sanitize food-contact surfaces to prevent contamination and bacteria growth. Dish machine found sanitizing at 0 ppm chlorine residual in sanitizing cycle; must ensure food-contact surfaces of dishes/utensils are effectively sanitized in solution of 50-100 ppm chlorine. Multiple containers of potentially hazardous, ready-to-eat foods in three-door and walk-in coolers found opened/made without make/open or seven-day discard date; must label all ready-to-eat potentially hazardous foods held 24 hours or more and discard within seven days. Handwashing sink was not producing hot water when turning hot handle tap; must ensure hot water is provided at every handwashing sink. Three-compartment sinks observed leaking from drain line with wastewater being collected in plastic containers underneath; must maintain plumbing in good repair.

Noncritical violations: Employee preparing foods/baked goods has full beard and has no restraint/cover to protect from incidental contamination of foods; must use beard restraints to cover hair when preparing foods. Dish machine and three- compartment sinks have buildup of debris/grease/food caked on interior of filter well, machine and sink basins; must frequently/effectively clean to prevent recontamination of food-contact surfaces. Non-food-contact surfaces of multiple pieces of dough press, cooling equipment interiors and handles and shelving/tables of prep areas/food storage have accumulation of food/grease/debris; must ensure frequent/effective cleaning of non-food-contact surfaces to prevent incidental contamination from occurring. Flooring of kitchen has large pieces of laminate missing, cracking and exposing layer underneath to debris harborage; must maintain physical facilities in good repair and restore surfaces to smooth, easily cleanable, durable, nonabsorbent characteristics. Walls of facility in food prep/storage and above warewashing is molded/cracked and chipping/peeling paint; must maintain physical facilities in good repair and restore surfaces to smooth, easily cleanable, durable, nonabsorbent and noncorrosive/contaminating characteristics.

Rice, noodles and bean sprouts need to be under time as a control; time shall be placed on product container stating when the food should be taken out of temperature control and must be discarded after four hours if not used.

Noncritical violations: Back door opened; all outer openings of a food establishment should be protected against the entry of insects and rodents.

Critical violations: Beverage container found without a lid; discontinue use of unapproved beverage containers in food preparation or warewashing area. Food employees did not change gloves after touching different surfaces and did not wash their hands; wash hands any time contamination could have occurred. Food employees touched ready-to-eat food with bare hands; no bare-hand contact with exposed, ready-to-eat foods or ice, use gloves, spatulas, tongs, deli tissue or other dispensing equipment. Change gloves every time dirty surfaces are touched or when you leave the food preparation area and return to work with food. Handwashing sink shall provide water at a temperature of at least 100 degrees through a mixing valve or combination faucet. Food, medicine, utensils and personal things are stored together. Potentially hazardous foods prepared and held for more than 24 hours must be clearly marked to indicate the date or day by which the food must be consumed or discarded; several prepared foods in walk-in cooler do not have a date-mark. Spray bottle with chlorine does not have a label. Pan with cooked foods are not covered in walk-in cooler.

Noncritical violations: Must have a sign or poster clearly visible that notifies food workers to wash their hands at all handwashing sinks used by food workers. Frozen meat and octopus thawing at room temperature; must use approved thawing method for time and temperature controlled safety foods. Food temperature measuring device shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperature. Bulk container with food (flour) lacks a label; food not in the original packages for use in establishment must be identified with the common name of the food. Ice machine lacks cleaning. The physical facility shall be cleaned as often as necessary to keep them clean; ceiling and floor are not clean.

Noncritical violations: Ice machine is not in working order; maintenance order placed. Walls under order area has holes; walls shall be fixed and in good repair.
