Washington County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Critical violations: Potentially hazardous foods and time- and temperature-controlled for safety foods hot holding must be 135 degrees or above; egg biscuits are hot holding below 135 degrees; if the temperature cannot be held the eggs must be thrown out after four hours.

Noncritical violations: Food temperature measuring device shall be provided and readily accessible; there is no thermometer to check hot holding or cooking temperature. Food that is not in the original packages for use in establishment must be identified with the common name of the food; powered sugar is not labeled. No chemical test strips for the sanitizer. No self-closing door for the men's bathroom. Small gap between filters in the vent hood.

McDonald's

4600 W. Sunset Ave., Springdale

Critical violations: Employees and a manager washed hands and did not turn the faucet off with a paper towel; employees shall wash hands using the procedures described in the food health code. Warewashing machine did not have proper sanitation levels at the time of inspection; the chemical sanitation shall be at 50 to 100 ppm. Chicken patties in hot holding were recorded at 115 degrees; potentially hazardous foods shall be held hot at 135 degrees or above. Multiple items have wrong discard date on products such as lemons, ham and bean salad; foods shall receive a date on seven days and then shall be discarded; repeat violation.

Noncritical violations: Wet wiping towels were found at the handwashing sink and the salad sink; hand towels shall be stored wet in sanitation buckets.

Cold-holding unit with raw meats and vegetables is holding food at 47 degrees; potentially hazardous foods shall be held at 41 degrees or below. Sanitation bucket does not have a label; all chemicals removed from the original container shall have the name of the chemical on the bucket.

Noncritical violations: Rice noodles in a bucket did not have a lid; all food products shall be covered to prevent accidental contamination, cardboard is being used as floor mats in the walk-in cooler and back room; items used in the kitchen shall be smooth and easily cleanable.
