Oct. 5

Sweet Stir

2420 U.S. 412 East, Siloam Springs

Critical violations: Employee did not wash hands when coming back into the unit prior to returning to beverage prep. Employee did not wash hands after handling money prior to preparing drinks. Milk in the reach-in cooler is at 45 degrees and half-and-half oat milk in the refrigerator is at 50 degrees. Repeat violation.

Noncritical violations: Wet wiping cloth is being stored on the countertop. Employee not wearing a mask when coming in contact with customers.

Oct. 6

E-Z Mart

387 S. Main St., Cave Springs

Critical violations: Employee washed baking trays but did not know about the sanitize step. Hot dog at 98 degrees and not marked with a time. Open packages of hot dogs and sausage on a stick in the walk-in cooler are not date-marked. Deli Express items in the refrigerator are not date-marked as needed.

Noncritical violations: Employee did not know where the test strips for checking sanitizer concentration are. Not found during the inspection.

Critical violations: None

Noncritical violations: Large amount of grease buildup and debris on vent hood.

The Creeks Golf Course

Critical violations: A case of onions sitting on the handwashing sink. Package of turkey at 50 degrees, cut tomatoes at 58 degrees. Unopened packages of luncheon meat taken out of the freezer not date-marked with the date removed from the freezer.

Noncritical violations: Employees encountering customers not wearing masks.

Critical violations: Milk at 46 degrees and small fridge needed to be defrosted.

Critical violations: Dirty dishes present in handsink at time of inspection. Raw eggs being stored above ready-to-eat produce in walk-in cooler.

Noncritical violations: Permit not posted in view of consumer.
