Addyson Paige Hoffman (Addy) passed away Monday. She is survived by her parents, Stephen Hoffman and Nicole Snyder; brother and sister, Ethan and Haley Snyder; grandparents, Mary Langford, Michael Hoffman, Don Bednar and Claudia Bednar.
