May 12

28 Springs Restaurant

100 E. University St., Siloam Springs

Critical violations: Employee handled raw meat and did not wash hands between glove changes. Drop guard on the ice machine has a build up on it.

Noncritical violations: Gnats at the bar and around the servers' stations. Glass in the door of the cooler in the very back prep station broke. Packing tape is keeping glass pieces from falling out of the frame.

Bakery Cuscatleca

2515 N. 17th St., Suite 301, Rogers

Critical violations: Employee did not wash hands when changing tasks from washing dirty dishes to working with food items. No paper towels available at kitchen handsink at time of inspection.

Noncritical violations: Paper bags of flour and sugar being stored directly on floor.

El Pueblito Mexican Restaurant

Critical violations: Open top glass of water on preparation counter. Soda machine has residue and is attracting insects (cockroaches). Raw beef and peppers in the walk in cooler have no date mark. Cockroaches in the soda machine cover. Beans were uncovered in walk-in cooler. The coolant fan above was leaking water. Food worker soiled apron stuffed into front of cleaned meat grinder. Multiple wiping clothes wet and soiled on preparation counter. Large industrial can opener covered in food debris.

Noncritical violations: None

Grub's Grille

Critical violations: Spray bottle of stainless steel cleaner stored on shelf with dry ingredients.

Noncritical violations: Retail food permit expired

The Café on Broadway

123 N. Broadway St., Siloam Springs

Critical violations: Case of raw bacon stored above packages of lunch meat.

Noncritical violations: Restroom door not self-closing. The spring loaded hinge does not appear to be working.

Ulianita's Mexican Restaurant

2515 N. 17th St., Suite 303, Rogers

Critical violations: Ready to eat rice and beans left in refrigerator uncovered. Ready to eat fajita chicken and veggies in refrigerator lacking date mark. Food discarded.

Noncritical violations: Burlap sacks of corn meal being stored directly on floor. Posted retail food permit is expired.

May 14

The Park House

201 W. University St., Siloam Springs

Critical violations: Bottom part of the prep table is not holding food at 41 degrees or below. The top part food temperatures are within allowable temperatures. This is a repeat violation from the previous inspection. Open container of hummus not date marked.

Noncritical violations: Permit posted is expired.

Critical violations: Cups stored in hand washing sink in kitchen. Cartons of shell eggs stored over milk in grocery reach-in cooler.

Critical violations: Hand washing sink unavailable, equipment on top of sink. Tamales and salsa not date marked.

Critical violations: None

Noncritical violations: Test strips not available for sanitizer at three-compartment sink. Build-up of grime on door/handle of sandwich prep station.

TNT Express

1196 Arkansas 59 South, Decatur

Critical violations: No paper towels for the hand washing sink. Chicken fingers, fried chicken, 115 degrees, left side of hot hold unit. Meat, butter, steak, 45 degrees. Unit turned to 38 degrees.

Callahan's Steak House

Critical violations: No sanitizer is being dispensed into the automatic dish washer at the bar. Automatic dishwasher is not dispensing sanitizer. Mac and cheese and shredded cheese in the drawer unit not at 41 degrees or below.

Noncritical violations: Soda nozzle at the bar has an accumulation of debris on the dispenser surface. When thawing vacuumed packed fish, the package must be opened to allow oxygen into package before thawing or removed from the package. Retail food permit posted but expired. Repeat violation.

Noncritical violations: Lid on chest type deep freezer is in need of repair, no longer smooth and cleanable. Rust inside chest type deep freezer, no longer smooth and cleanable.

Critical violations: Not all food in the preparation refrigerator is at 41 degrees or below. Unit may be too full not allowing for proper air flow. Some items were sitting out preparing orders for customers. Most items are not date marked as needed.

Noncritical violations: Wet wiping cloth not stored in sanitizer.
