Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

Priority violations: Quaternary ammonia sanitizer was over 400 ppm; must be 200 ppm per label (corrected).

Parkside Public

112 S. First St., Rogers

Priority violations: Peppadew peppers 44 degrees in prep top near grill; dressing 44 degrees in refrigerator.

Priority foundation violations: Prep table top and refrigerator are not keeping foods at 41 degrees or below.

Priority violations: Automatic dishwasher was at 0 ppm for chlorine sanitizer; must be 50 to 100 ppm (corrected).

Kirksey Middle School

Priority violations: Evidence of mice (droppings) observed in the mop utility room.

Core violations: Gap between floor and wall in the mop utility room.

Priority violations: Using quaternary ammonia in automatic dishwasher; requires chlorine sanitizer 50 ppm per label.

Priority violations: No date marking on items as needed.

Eureka Pizza

202 Highway 412 W., Siloam Springs

Priority violations: Unlabeled spray bottle at three-compartment sink. Small hot box is not holding chicken wings at the correct temperature; adjust as needed. Employee did not wash hands properly or long enough.

Core violations: Scented bleach; use only approved bleach and concentration. Torn gaskets in preparation table (work order submitted). Lid on prep table is coming apart; insulation in lid is exposed and lid is not easily cleanable (work order submitted). No test strips for sanitizer being used; employees said they have not used them.

Dollar General

200 Highway 412 W., Siloam Springs

Priority violations: Some packages of lunch meat expired.

Core violations: No thermometer in meat/cheese case. Thermometer in milk case is reading 44 degrees; adjust as needed.

March 25

The Station

400 S. Gentry Blvd., Gentry

Priority violations: Maintain hot food at 135 degrees or above.

Core violations: Replace the lid on the freezer; duct tape is not an approved method.

Miss Mary's

486 S. Gentry Blvd., Gentry

Priority violations: Items not date marked as needed. Employee handled ready-to-eat foods with bare hands. Employee came back into kitchen and did not wash hands. Dish washer did not wash hands after handling dirty dishes before handling clean dishes.

Core violations: Employee wearing finger nail polish and not wearing gloves. Condensation leak in freezer; do not store open items under leak. Effective hair restraints required for food employees.

Sunrise Cafe

136 S. Gentry Blvd., Gentry

Priority violations: Employee did not wash hands after handling raw meat (corrected). Employee did not wash hands with soap (corrected). Bag of raw chicken meat stored on the same tray with open ready-to-eat foods; all food destroyed. Refrigerator is not holding food at 41 degrees of below. Preparation table is not holding food at 41 degrees or below. Demonstration of knowledge out of compliance due to the above priority violations.

Core violations: Clean exhaust vent/hood all the way to the roof and down. Clean cabinets grill under equipment and inside all cooling units.

Sonic

316 S. Gentry Blvd., Gentry

Priority violations: Employee did not wash hands after handling raw meat (corrected).

Core violations: Drop guard in ice machine is rusty; replace as needed. Condensation leak in freezer; do not store open food under leak. Excessive grease build up under grill. Clean inside the fryer cabinets.
