West Fork woman in van dies in crash

— A West Fork woman died Wednesday in a two-vehicle accident on Arkansas 170, two miles west of Interstate 540, according to a preliminary Arkansas State Police fatality accident report.

About 7:20 a.m., James Ennis, 43, of West Fork had stopped his trash truck on the shoulder and partially in the eastbound lane of Arkansas 170 and moved to the rear of the truck to collect trash, the report states. Regina L. Couch, 35, of West Fork, driving east on the highway in a 1991 Plymouth van, failed to see the truck in time to stop, according to the report.

Couch swerved to the left, struck Ennis and then hit the left rear side of the trash truck.

Wylladen D. Richards, 54, a passenger in the van, died in the accident, the report states. Ennis and Couch were taken to Washington Regional Medical Center. The extent of their injuries and conditions were not available Wednesday.
