Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Priority violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Priority foundation violations are defined as items relating directly to factors that lead to food-borne illness such as training, equipment plans and documentation and must be corrected immediately.

Core violations are defined as items that relate to maintenance of food operations and cleanliness.

June 15

Theo's

3300 Market St., Rogers

Priority violations: Prep table No. 1 is not holding food at 41 degrees or below. Mashed potatoes still 114 degrees in the walk-in; cool foods properly (corrected). Walk-in No. 2 is not holding food at 41 degrees or below; food moved to other cooler as needed.

Ruth's Chris Steakhouse

3529 Pinnacle Hills Parkway, Rogers

Core violations: Maintain hollandaise at 135 degrees or above or follow the recipe guidelines as written.

The Big Chill

3000 Pinnacle Hills Parkway, Rogers

Priority violations: Front handwashing sink is blocked with a table and bus table; sink must be accessible at all times. Demonstration of knowledge, employees must be trained properly. Drawer prep table unit is not holding food at 41 degrees or below. Items not date marked as needed. Some items exceed the allowable number of days.

Core violations: Excessive condensation leak in the freezer; ice building on food and box. Cup in blue cheese dressing. PVC pipe is not approved food contact surface; bottle holders in ice are not food-grade items. Ceiling tiles missing in back area. Storage shelf by dish area is rusty and paint is peeling; no longer smooth and easily cleanable.

The Grove

2600 S. Pleasant Crossing Blvd., Rogers

Priority violations: Prep table unit No. 1 is not holding food at 41 degrees or below. Demonstration of knowledge out of compliance. Prep table unit No. 2 is not holding food at 41 degrees or below. Employees not washing hands prior to glove use or between glove changes or as needed. Mixed cut greens on ice not held at 41 degrees or below; ice is only effective when surrounding the product completely. Pan of raw meat (steaks) stored on same pan as deli meats (corrected); pans of raw meats stored on top of kegs. Rice still 70 degrees from preparation earlier today; cool foods properly.

Core violations: Bar: Soda gun holder broken/gone; storing in ice bin. Handwashing sink falling off the wall. No test strips for sanitizer. Replace ceiling tiles as needed so they are smooth and easily cleanable. Floor wall seal is peeling. Drips hitting the floor, not floor drain.

June 17

Super 8

509 Hospitality Lane, Lowell

Priority violations: Employee was not able to explain how to make sanitizer, said was not trained how. Employees must be trained properly on all aspects of food service.

Core violations: Freezer lid inside insulation guard is coming apart; repair or replace; tape is not an approved repair method; insulation could fall onto food items in freezer. Storing dishes on towels; dishes must be put on something that is nonabsorbent and easily cleanable to air dry; additional drying racks could be used. No plugs for sinks; plugs are necessary to wash and sanitize dishes.

Wagon Wheel Cafe

4080 N. Thompson St., Springdale

Priority violations: Food being prepared in back store room; back store room does not have a handwashing sink; there is a door separating the kitchen from the store room. Employee handled ready-to-eat food with bare hands. Beans do not appear to have been cooled properly, some pans 41, 47 or 51 degrees; do not use beans. Clean interior of ice machine.

Core violations: Employees did not wash hands after cracking raw eggs.

Gusanos of Rogers

2603 W. Pleasant Grove Road, Rogers

Priority violations: Prep table (pizza) No. 3 is not holding food at 41 degrees or below; sausage destroyed as needed. Some employees not washing hands between task changes or as needed. Storing food items in front of the fan units in No. 2 and No. 3 prep tables; do not block the air flow of units.

Core violations: No test strips for sanitizers in back of bar; employees not aware where they would be. Sanitizer concentrations must be checked daily and each time made at bar. Water pooling under ice well; check for leaks. No thermometers in numbers two and three prep tables; check temperatures of units and food daily.

McAlister's Deli

900 S.E. Walton Blvd., Bentonville

Core violations: Grout is eroded in kitchen prep areas between tiles; repair floor so it is easier to clean.

Papa Murphy's

2600 Pleasant Crossing Drive, Rogers

Priority violations: Cookie case/cheesy bread cooler is not turned on; food items not at 41 degrees or below. No sanitizer on location. Employees did not wash hands prior to glove use (corrected).

Core violations: No test strips on location.

Chick-Fil-A

2601 W. Pleasant Grove Road, Rogers

Priority violations: Raw shell eggs stored above lemonade containers in walk-in (corrected).

Core violations: Products/boxes on floor in walk-in; items must be stored at least 6 inches off the floor (corrected).

Golden Corral

2605 Pleasant Crossing Drive, Rogers

Priority violations: Dishwasher is not reaching proper sanitizer temperatures; manually sanitize until repaired.

Core violations: Condensation leak in freezer walk-in; do not store open items under leak area. Repair or patch broken tiles as needed in various places. Leak or hole in drain pipe of dishwasher.

Econolodge

3609 Moberly Lane, Bentonville

Core violations: Wallpaper behind three-compartment sink is peeling and moldy; repair. Make wall behind sinks smooth and easily cleanable.

Shipley's Donuts

271 U.S. 412, Siloam Springs

Priority violations: Maintain gravy at 135 degrees or above. Employees not washing hands as needed or properly.

Core violations: Using paint brushes for basting or brushing egg and butter; brushes must be food grade. Not keeping time logs on kolache; destroyed as needed. Store wiping cloths in sanitizer.

Cathy's Corner

1910 E. U.S. 412, Siloam Springs

Priority violations: Employee handled ready-to-eat foods with bare hands. Pulled pork at 102 degrees; may not have gotten reheated hot enough prior to putting in hot holding (reheat to 165 degrees). Clean interior of ice machine. Milk and dessert cooler is not holding food at 41 degrees or below; milk at 45 degrees.

Core violations: Comet cleaner is not labeled for restaurant use on food-contact surfaces; clean and then sanitize surfaces. Server used cup to scoop ice directly; use scoop utensil not the glass itself.

Kung Fu Eggroll

24658 Lamphear Road, Gentry

Priority violations: Refrigerator is not holding food at 41 degrees or below. Container of raw meat stored on top of vegetables (corrected).

Suzy's on Dogwood

881 S. Dogwood St., Siloam Springs

Core violations: Clean interior of two-door white refrigerator.

June 22

Shipley's Donuts

2119 W. Walnut St., Suite A, Rogers

Priority violations: Kolaches taken out of oven and put on racks at 7:30 a.m. were not labeled with a time to pull or discarded since they were not held in temperature control. You have opted to use time to limit bacterial growth. This requires you to have a written procedure for labelling these foods (repeat).

Priority foundation violations: Only handsink in rear part of kitchen is blocked with empty basket. Employee stated that it is not working and that is why it did not have soap or paper towels.

Core violations: Blueberry cake donut flour was stored on floor with scoop in it; store 6 inches off the floor to prevent contamination.

Kum & Go

1701 W. New Hope Road, Rogers

Priority foundation violations: Cookies and other baked goods available for customer self-service at checkout do not have ingredients information listed by weight and have any other major allergens clearly identified.

McDonald's

4208 Pleasant Crossing Blvd., Rogers

Priority violations: Register operator dumped fries into the hopper (dispenser) and then touched them with her unwashed, bare hands. Time as a control is not being followed; items not time or date marked as needed. Manager handled money and did not wash hands prior to putting on gloves or task change. Sanitizer is not strong enough, not reading any concentration.

Core violations: Significant condensation leak in walk-in cooler; water and ice all over product; do not store open containers under leak (service call made). Employees, including managers, must wear effective hair restraints.

John Q. Hammons Center

3201 Pinnacle Hills Parkway, Rogers

Priority violations: Clean interiors of both ice machines at servers' station.

Core violations: Condensation leak in freezer. No test strips for quaternary ammonia sanitizer (corrected).

La Carreta Mobile No. 2

521 S. Bloomington St., Lowell

Priority violations: Maintain cold food at 41 degrees or below; food in prep table and black fridge and white refrigerators. Pump for handwashing has to be working on demand, should not have to turn on breaker each time handwashing is necessary. Pico is not held at 41 degrees or below; when ice is used, the ice must be at or above the level of the food. Employee handled raw meat, took off gloves and was going to put on new ones without washing hands; handwashing is required between glove changes.
