Benton County Restaurant Inspections

Information is from state Health Department records. Restaurants are listed in order of inspection date. All reports are from regular food service food store inspections unless otherwise noted.

Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately.

Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Aug. 26

Las Palmas

200 W. Poplar St., Rogers

Critical violations: Chili meat 48 degrees in large container in walk-in cooler; did not cool properly; must cool from 135 to 70 degrees or below within two hours, then cool from 70 to 41 degrees or below within four additional hours. Beef fajita 109 degrees and chicken fajita 129 degrees in hot well; must be kept at 135 degrees or above after cooking in hot holding.

Noncritical violations: Inside ceiling of ice machine is not clean.

Fast Lane Entertainment

Critical violations: None

Noncritical violations: Quaternary ammonia sanitizer is too strong. Employee food stored with establishment food; store employee food in such a way to prevent possible contamination (corrected). Missing handle on the upright refrigerator; string is not an approved repair material. Floor by the ice machine/beer cooler is not clean. Monitor salad cooler to ensure that it will maintain food at 41 degrees or below. Establishment says the exhaust vent cleaning has been scheduled.

Aug. 28

Hawg City Grill

Critical violations: Spray bottles of pre-mixed Clorox cleaner with bleach is not approved for food contact surfaces or food preparation areas.

Noncritical violations: Door gasket on meat reach-in cooler is torn and no longer in good repair. Condensation leak in freezer; do not store open food under leak. No light in walk-in freezer.

Casey's General Store

1531 S. Eighth St., Rogers

Critical violations: Using vinegar and water as a sanitizer on food contact surfaces such as cutting boards and prep table; must either use quaternary ammonia per label or chlorine at 50 to 100 ppm. Meatballs in sauce 125 degrees, gravy 128 degrees, barbecue 127 degrees in hot well; must be kept at 135 degrees or above; meatball in sauce and barbecue were reheated to 200 degrees; gravy was discarded (corrected). Missing label on spray bottle containing chemical.

Noncritical violations: Container being used for scoop in pizza prep does not have a handle; scoops must be handled with handle stored out of product. Ice scoop was being stored on top of ice machine between uses; not a clean sanitized surface. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in accordance with establish regulations.

Miss Mary's

468 Gentry Blvd., Gentry

Critical violations: Wash hands any time contamination could have occurred, when changing tasks. Dishwasher did not wash hands between handling dirty and clean dishes; employee just rinsed off dish washing glove. Raw meat stored above ready-to-eat foods in the reach-in cooler and the walk-in (corrected).

Noncritical violations: Salad make table in use with open food, while the floor drain was backing up in the vicinity; food moved to closed cooling unit until backup corrected. Not all food workers wearing effective hair restraints. Some dishes drying directly on cloth towels. Floor drain backing up into the salad preparation area. Condensation leak in the walk-in cooler. Permit posted but expired.

Backyard Burgers

Critical violations: Require food employees to wash their hands using the procedure in the Arkansas Rules and Regulations Pertaining To Food Establishments section 2-301. Employee did not lather hands long enough prior to rinsing off soap. Wash hands any time contamination could have occurred, when changing tasks and as described in the Arkansas Rules and Regulations Pertaining To Food Establishments section 2-301. Employee went from mopping the floor to handling the fry basket. A handwashing sink shall be maintained so that it is accessible at all times for employee use; storing brooms and dust pans in front of handwashing sink. Condensation leak in the walk-in freezer had dripped onto open food and on the freezer floor. Make line refrigerator is not holding food at 41 degrees or below; food still in temperature was moved to walk-in cooler. Salad cooler is not holding food at 41 degrees or below; food moved to walk-in cooler. Using time as a control on tomatoes on make line; items not marked with time as needed.

Noncritical violations: Foot pedal was off the cold control on one handwashing sink (corrected). Water leak in faucet at three-compartment sink. Constant water leak from the sprayer at the back preparation sink. Condensation leak in the walk-in freezer. Constant water leaking from the refrigerator on the make line. Drain for soda machine has buildup in drain; clean regularly as needed. Knives being stored between pans on hot holding line. Ensure all employees know the proper steps for washing utensils and equipment; wash with soap and water, rinse off soap, sanitize and air dry.

Loyd's Little Land

1723 N. Concord St., Lowell

Critical violations: Milk was 63 degrees and 56 degrees coming back from the rooms; milk must be at 41 degrees or below.

Noncritical violations: Employee food stored with establishment food; separate and identify employee food area. Food workers shall wear hair restraints, beard restraints and clothing that covers body hair. Reusing single-use containers for food service. Three-compartment sink has sticky residue in the corners and drain of the sink; sink is not being kept clean.

Fratelli's Wood Fired Pizzeria

Critical violations: Not all food in the prep table is holding at 41 degrees or below. Items not date marked as needed.

Noncritical violations: Clean under and around pizza oven. Gasket torn on preparation table door. Permit posted but expired.

Southside Elementary School

Critical violations: None

Noncritical violations: Wooden shelves in walk-in freezer No. 2 are not smooth, easily cleanable; seal raw wood or replace. Condensation leaks in freezer No. 1. Ice build up along the door of walk-in freezer No. 1.

Sweetwater Tavern

201 W. University St., Siloam Springs

Critical violations: Nozzle holder in the bar area is not clean. Spilled beer inside kegerator. Alfredo and Marsala sauces were not at 41 degrees or below; possibly due to improper cooling; other food items checked in the same cooler were below 41 degrees; food destroyed as needed.

Noncritical violations: The premises shall be maintained free of insects, rodents and other pests and controlled to eliminate their presence on the premises. Fruit flies in the bar area. Shelves and dividers made out of cardboard; cardboard is not an approved construction material for food equipment and container storage. Towels under drain rack are not clean (corrected).

Critical violations: Active managerial controls to maintain food safety not observed during time of inspection; onsite tutorial given to manager. No bare-hand contact with food or ice; use gloves, spatulas, tongs, deli tissue or other dispensing equipment. Manager observed processing heads of lettuce with bare-hand contact; onsite product disposal. Entire contents of pizza make table 55 to 68 degrees; items placed on four-hour time log. Pizza make station items kept out of temperature during operation must be kept on four-hour time log with documentation.

Critical violations: Ice bucket is not clean; wash rinse and sanitize as needed. Drop guard inside the ice machine has a slimy film on it; wash and sanitize frequently. Drop guard inside ice machine has been painted; paint is chipping and peeling; paint is not an approved repair material for this application; replace drop guard with an approved material for this application (10 days).

Noncritical violations: Duct tape used on the service counter; duct tape is not an approved repair material for this application. No test strips on location to check the concentration of bleach sanitizer; concentration should be 50 to 100 ppm. Dried milk spill and other food items under the shelves in the walk-in; clean frequently enough to prevent the accumulation of food. Permit posted but expired.

Pour Jons

223 N. Wright St., Siloam Springs

Critical violations: Discontinue sale or use of shelled eggs that are not clean, sound and exceed the restricted egg tolerances for U.S. Consumer Grade B standards. Bag of sliced turkey stored above the load line in the prep table. Eggs still 105 degrees in the prep table. Ensure foods are cooled properly prior to placing in cold holding.

Noncritical violations: Clean interior of the small ice unit under the bar to remove the accumulation of debris.

Noncritical violations: Equipment for cooling and heating food, and holding cold and hot food, shall be sufficient in number and capacity to provide food temperature in accordance with rules and regulations. Food temperature measuring device shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperature.

Allen Elementary School

Critical violations: Employee washed hands at three-compartment sink; employee grabbed clean gloves prior to washing hands. Package of raw bacon stored above ready-to-eat foods in the walk-in cooler; raw bacon should be stored below all ready-to eat-foods. Food in prep table is not being held at 41 degrees or below; unit appears to be frozen up; food destroyed as needed. Sanitizer concentration too strong; should be between 50 to 100 ppm.

Noncritical violations: Personal cooler of ice stored in the prep table with establishment food. Food workers shall wear hair restraints (corrected). No test strips on location to check sanitizer concentration.

Casey's General Store

Critical violations: Using vinegar and water to sanitize prep top and other food contact surfaces; must either use quaternary ammonia per label or chlorine at 50 to 100 ppm (repeat, corrected). Barbecue beef 128 degrees and meatballs 109 degrees in hot well; must be kept at 135 degrees after cooking to 165 degrees (repeat).

Estrada's Mexican Restaurant

Critical violations: None

Noncritical violations: The handwashing sink may not be used for food preparation or utensil washing or a service sink or in any other manner other than handwashing. Broken ice bin door repaired with absorbable cotton string.

El Farolito Tex-Mex Restaurant & Bakery

415 W. Centerton Blvd., Centerton

Critical violations: Make table ambient temperature 64 to 70 degrees; approximately 5 pounds of pico, raw chicken, salsa and leafy greens disposed of onsite. Equipment for cooling and heating food, and holding cold and hot food, shall be sufficient in number and capacity to provide food temperature in accordance with rules and regulations. Make table out of service until serviced and cooling properly,

Noncritical violations: Discontinue consuming food, use of unapproved beverage containers or use of tobacco in food preparation or warewashing area. Employee beverage containers must have lid and straw.

Noncritical violations: Provide a supply of individual disposable towels or a continuous towel system that supplies the user with a clean towel or a heated-air drying device at each handwashing sink in food preparation, toilet and warewashing areas. Washing hands in three-compartment sink.
