Dr. Timothy David Chase

Dr. Timothy David Chase, age 53 of Monticello, went to be with his Lord Friday, April 15, 2016. He was born November 24, 1962 in Monticello, AR to the late David Lee Chase, Jr. and Mary Agnes Bailey Chase. Tim, as he was affectionately known, was very involved in his community, church and dental association. He was a 1981 graduate of Monticello High School, a 1985 honor graduate of the University of Arkansas at Monticello, and received his Doctor of Dental Surgery from the University of Tennessee in 1989. He has been in dental practice in Monticello since 1991. His professional affiliations include member of the International College of Dentists, American Dental Association, former President of the Southeast District Dental Society, board member of the Arkansas State Board of Dental Examiners, former President of the Arkansas State Dental Association, Medicaid Task Force, alternate delegate to the ADA Annual Session, and a charter board member of the Arkansas Mission of Mercy where he was the Local Coordinator. Tim was currently a sitting Monticello City Council Alderman elected in 2004 and was involved in many other community activities including being an active member of the First Baptist Church, former little league baseball coach, Chamber of Commerce board member, Industrial Development Committee member, past Monticello Girls Softball Association coach, board member of the Monticello Schools Foundation, member and past-president of the Monticello Rotary Club. Tim was an avid Monticello Billie, UAM Boll Weevil and Arkansas Razorback fan. Other than his civic and professional involvement, his greatest love was his family. In addition to being a wonderful husband to Paige, Tim always sacrificed his time for his two daughters, Summer and Madisyn. He never missed a softball game, dance recital, piano recital, swim meet or gymnastics meet and was always ready to give his time to support his girls. Survivors include his wife, Leslie Paige Maxwell Chase of Monticello; and two daughters, Summer Elizabeth Chase and Madisyn Olivia Chase both of Monticello. Funeral service will be 3:30 p.m. Monday, April 18, 2016 at First Baptist Church with Rev. Bruce Venable officiating. Burial will be in Oakland Cemetery. Family visitation will be from 6:00-8:00 p.m. Sunday evening at Stephenson-Dearman Funeral Home.
